ABSTRACT
Introdução: O uso de piercing oral tem despertado o interesse dos profissionais da Odontologia devido à associação com diversas complicações locais e sistêmicas. Objetivo: Este trabalho objetivou realizar uma revisão sistemática da literatura sobre riscos e complicações associadas ao uso de piercings orais e periorais. Métodos: Foi realizada uma pesquisa nas bases de dados MEDLINE, Google acadêmico e LILACS utilizando-se os seguintes critérios: artigos publicados na íntegra, no período de 2000 a 2016, em inglês, português e espanhol, case report, classical articles e reviews sobre o tema utilizando os seguintes descritores: body piercing, risks management, complications, risk e seus mesh terms. Resultados: Foram incluídos um total de 39 artigos relevantes que cumpriam com todos os critérios. Foi observado que os locais mais frequentemente utilizados para a colocação de piercing oral são os lábios e a língua. As complicações incluem inflamação, infecções locais e sistêmicas, lesões mucocutâneas, lesões vasculares, trauma dental, problemas periodontais, reações de hipersensibilidade, disfonia, deglutição e aspiração do piercing, e até morte etc. Conclusão: Conclui-se que o cirurgião dentista deve exercer um papel importante na orientação da população em geral sobre os riscos e as complicações associadas ao uso de piercing oral, visando evitar tal prática ou minimizar ao máximo os riscos e as complicações naqueles indivíduos que os possuem. Introduction: The use of oral piercing has aroused the interest of dental professionals because of its association with various local and systemic complications. Objective: This study aimed to perform a systematic review on risks and complications associated with the use of oral and perioral piercings. Methods: We searched the MEDLINE, Google Scholar and LILACS databases using the following criteria: articles published in full text, from 2000 to 2016 in English, Portuguese and Spanish, case reports, classic articles and reviews about the topic using the following descriptors: body piercing, risk management, complications, risk and their mesh terms. Results: 39 relevant articles that fulfilled all the criteria were included. It has been observed that the most frequently used sites for an oral piercing placement are the lip followed by the tongue. The complications included inflammation, local and systemic infections, mucocutaneous lesions, vascular lesions, dental trauma, periodontal problems, hypersensitivity reactions, dysphonia, swallowing and aspiration of the piercing, and even death etc. Conclusion: Its was conclude that the dentist can play an important role in guiding the population about the risks and complications associated with the use of oral piercing, aiming to avoid such practice or minimizing the maximum the risk and complications in those individuals who possess them. O presente trabalho tem como objetivo discorrer sobre o uso do piercing lingual e suas consequências na cavidade bucal, especificamente nos tecidos periodontais. É prevalente em ambos os sexos e com faixa etária diversificada, destacando, no entanto, maior entre adolescentes e adultos jovens. Essa realidade é exemplificada por meio de dois pacientes, atendidos no ambulatório da disciplina de Periodontia, da FOP/UPE. O exame clínico revelou várias alterações nos tecidos periodontais, tais como: inflamação gengival, cálculo dentário, recessão gengival e espaçamento entre os dentes. Como fator complicador, os pacientes em questão se negaram a retirar os piercings, e, desse modo, o tratamento planejado não pôde ser executado. Conclui-se destacando a importância do cirurgião-dentista na conscientização do paciente sobre os danos que esse tipo de adorno pode ocasionar nos tecidos da cavidade bucal.
This paper aims to discuss the use of tongue piercing and its consequences in the oral cavity, specifically the periodontal tissues. It is prevalent in both sexes and diverse age, emphasizing, however but higher among adolescents and young adults. This reality is exemplified by two patients treated at the outpatient clinic of discipline of periodontics, FOP/UPE. Clinical examination revealed several changes in tissues, such as gingival inflammation, dental calculus, gingival recession, and spacing between the teeth. As complicating factor in question patients refused to remove the piercing, and thus the planned treatment could not be executed. Given the relevance of the subject, concludes by highlighting the importance of the dentist in patient awareness of the damage that type of decoration can cause the tissues of the oral cavity.

El objetivo principal de la realización de la presente investigación fue evaluar la presencia de Recesión Gingival en el sector antero inferior en estudiantes que utilizan Piercing Lingual o Labial, de las diversas Facultades de la Sede Central de la Universidad de El Salvador en el período de Marzo-Abril de 2009; estableciendo inicialmente el tipo de recesión que presentaron los pacientes, con base en la clasificación de Miller que va desde un estado leve (Clase I) a un estado severo (Clase IV). Este estudio se llevo a cabo en las instalaciones de la Facultad de Odontología, de la Universidad de El Salvador, contando con una población de 53 estudiantes de las Facultades de Ciencias y Humanidades, Ingeniería y Arquitectura y la Facultad de Química y Farmacia, los cuales presentaban piercing Lingual o Labial, en quienes se determino la localización y el período de tiempo de uso de dicho aditamento, estableciendo tres períodos: menos de 3 meses, de 3 a 6 meses y mayor a 6 meses. Del total de pacientes evaluados, la mayor prevalencia de recesión gingival fue la clase I, lo cual fue más notable en la población masculina y mayormente asociado con el uso del aditamento en la zona lingual; estos hallazgos ocurrieron principalmente durante un periodo de uso mayor a 6 meses.
The main objective of carrying out this research was to evaluate the presence of Gingival Recession in the antero-inferior sector in students who use Lingual or Labial Piercing, of the various Faculties of the Central Headquarters of the University of El Salvador in the period of March- April 2009; Initially establishing the type of recession that the patients presented, based on Miller's classification that ranges from a mild state (Class I) to a severe state (Class IV). This study was carried out in the facilities of the Faculty of Dentistry, University of El Salvador, with a population of 53 students from the Faculties of Sciences and Humanities, Engineering and Architecture and the Faculty of Chemistry and Pharmacy, which They presented lingual or labial piercing, in whom the location and period of time of use of said accessory were determined, establishing three periods: less than 3 months, 3 to 6 months and more than 6 months. Of the total of patients evaluated, the highest prevalence of gingival recession was class I, which was more notable in the male population and mostly associated with the use of the abutment in the lingual area; These findings occurred mainly during a period of use greater than 6 months.

A arte do corpo (body art) vem ganhando popularidade atualmente. O uso de piercings é uma forma de expressão desta arte e está bem difundido principalmente entre os jovens. A cavidade oral e, mais especificamente, a língua, é um dos locais mais comuns. O presente trabalho visa relatar um caso clínico de uma paciente usuária de piercing na língua que apresentou recessão gengival e abscesso periodontal. O uso do piercing na língua pode trazer muitas consequências indesejáveis para o periodonto e para a dentição, por isso os odontólogos devem estar cientes para alertar e tratar esses pacientes.
